The government have recently issued guidance for restaurants, pubs, bars and takeaways reopening from 4 July 2020. You can view this guidance on this document.
The government has also issued Guidance for temporary alcohol licensing for the off sales of alcohol.

Application to vary a club premises certificate
Once you have completed the form you will need to send it to us. You can do this by scanning the two forms and emailing them to us. Or you can post them to us at:
