About our GP Link Worker Service
Do you have a long-term health condition that stops you being as active as you would like? Are you interested in improving your physical and mental health through physical activity? If so, we are encouraging Hastings residents (aged 18+) to speak to their GP and ask for a referral to our Active Hastings GP Link Worker service.
Our Active Hastings GP Link Workers are helping to create a link between the NHS and the community; helping guide patients into being more physically active by creating a support system into exercise and helping them take control of their own health.
Once a patient is referred to our service, they will be contacted by a qualified exercise referral specialist from our team, who will speak to them about their current medical conditions, medication, general health and wellbeing. Based on this information recommendations and support will be provided to help increase physical activity levels.
The Active Hastings GP Link worker roles are funded by Hastings Primary Care Network.
Information for GPs
Who can be referred?
- Anyone aged 18+ who would benefit from being more physically active
- People with medical conditions, if their condition is deemed clinically stable and/or under control.
GPs and healthcare professionals can download referral forms from DXS.
