I'm leaving or have just left hospital
I'm currently in hospital
Are you currently in hospital and are due to leave soon? Do you live in Hastings but have nowhere to go to when you leave hospital? You can ask hospital staff to make a Duty to Refer application to the council so we can help you.
Please be aware that Duty to Refer applications can only be completed and submitted by a health or social care professional.
When we receive your application, we will contact you to discuss your housing situation. A Housing Options Officer may visit you in hospital, or book an appointment to see you when you leave hospital.
Alternatively, you can call the council on 01424 451100 to book an appointment to see a Housing Options Officer. They will then talk to you about your housing situation and how they may be able to help and advise you further.
I have just left hospital
If you have just left hospital and a Duty to Refer application has not been made on your behalf please call us on 01424 451100.
I need support when I return home from hospital
If you're about to be discharged from hospital and are concerned that you may have difficulty managing when you return to your home, there are services that can help you with the support you need to stay in your home.
Adult Social Care can carry out an assessment to:
- Find out what kind of care you need
- Talk to you about what care options are available
- Check you aren't missing out on any help with costs
- Help you manage your current and future care costs
This may result in you receiving professional care at home, being referred to an Occupational Therapist who could assess whether your home is suitable for adaptations, or getting access to a short term care home.
