I would like to request the following, if possible:

1. Does the County Council reimburse NHS health checks in any service specifications with General Practices within your council?

2. If Yes, what percentage of health checks were performed in 2023/24 financial year? Please may I have this as two columns: (a) GP Practice Name + ODS code (Unique identifier), (b) Percentage of Health Checks performed in 2023/24.
