Whilst I acknowledge that your Local Authority is not responsible for the provision of Public Health services, can you please confirm if your Local Authority provides any form of grants or funding, or commissions any projects of any size that are intended to support communities, groups or individuals to lead healthier lives?: Yes or No.

If so, can you please confirm the following details of the lead individuals (Team Leaders, Directors, Department Heads, Programme Managers, and so on) responsible for the management of those grants, funding or commissioning:

b. Job Titles

c. Email Addresses
