Could I please ask you to provide the following information in relation to food businesses that have recently registered with your local authority Environmental Health team?

Could you please provide me with details of all food businesses that made an application to be registered with you in accordance with Regulation (EC) No 852/2004 Article 6(2) between the 1st November 2018 and the 5th February 2019?

Can you please return the information into an excel spreadsheet as follows:

Column1: Trading Name of Business

Column2: Full Address of Establishment

Column3: Postcode of Business

Column4: Please indicate the "Type of Food Activity" that the business is registered for

Column5: Business email address as registered
