It is requested that the following information is provided where your organisation (or a third party such as a local district/borough Council acting on your behalf) has contracted with a catering services company for the provision of a group school meals service that is either delivered to state schools or produced on site in state schools.

Such contracts are for a group of schools (Primary, Secondary or a mixture of both) where a group is defined as two (2) or more schools
